Brains don't have pain receptors, so you could mess with someone's brain while they were awake. You could severely damage their memory, cause them to be...well. I guess they'd die. xD

That assumes you know exactly what part of the brain does what. Not all of it is memory stuff. Not even most of it.

If you want to get really sadistic, cut the motor branches of the spinal and leave the sensory ones intact. It's a little difficult, but if you do it right, the body is your playground for suffering while they can't do anything about it. Of course, cut after the neck vertebrae. Before that, you kill them.
