Maybe each human being lives in a unique world, a private world different from those inhabited and experienced by all other humans. . . If reality differs from person to person, can we speak of reality singular, or shouldn't we really be talking about plural realities? And if there are plural realities, are some more true (more real) than others? What about the world of a schizophrenic? Maybe it's as real as our world. Maybe we cannot say that we are in touch with reality and he is not, but should instead say, His reality is so different from ours that he can't explain his to us, and we can't explain ours to him. The problem, then, is that if subjective worlds are experienced too differently, there occurs a breakdown in communication ... and there is the real illness.
As a person who has written stories about a series of characters who are the personification of madness, I think one thing I should start off with is that there are different forms of madness. Whenever I wrote my characters, I distinguished and categorized these as just plain madness, like what we see from the Mad Hatter, and then true insanity. It was an interesting experiment all around because the first personification wasn't all that mad at all until her one and only child died. Prior to that point in her life, the first incarnation was rather calm and actually somewhat shy around others. She was just a normal person, not someone we would say is "mad". Then, the second personification of madness (whom you all know as Madder) was a bit off his rocker. He was more towards the insanity form of madness from the get go, having witnessed his parents be murdered in front of him and then being forced to kill their killer in order to ensure his survival when he was just a little kid. From then on, he heard voices in his head coaxing him to do the same to others, but he was able to resist their influence for the most part. It wasn't until the voices were no longer voices, but rather his own thoughts that he slipped up and went completely insane. Then, the third and present incarnation is more along the lines of what you see in the Mad Hatter. She's always happy, never sees the bad in a person, and has a real naivety to her. She is almost like a delicate flower than can be easily bruised, but everyone is doing their best not to touch her.
I think these cases can be applied to kind of what everyone else is saying. The first incarnation's reality wasn't really different from the world that those around her experienced day by day. It was just a common vision to them all. The second incarnation's reality was filled with hate and torment, more so than those around him, so his overly aggressive responses made him seem off his rocker. And then the third incarnation's reality is all sugar plums and gum drops, making her an overly happy person. She doesn't realize how much hate, sadness, and negativity there is in the world, so she's mad by her naivety because everyone else knows the truth, but she does not.
