For the form, what does the 'What did they give or take' section mean? Also, is 'CYCLONE intern' the rank and group, and how do we know which team they would be on?

That form is just general form for all villains and rivals. Its help keep track of those that pass through the story. Some villains will give up money or an item so Id like to keep track of that. If one of the sections does not apply simply put N/A for now. They may get filled in later in the story.
